This page is used to request an e-mail that contains your Web userId and password when they have been forgotten. To initiate the e-mail, enter your SSN, select a verification question from the drop down menu, enter the corresponding verification answer and select the submit button. You will receive a message indicating that an e-mail containing your userId and password will be sent to you.
